Health Questionnaire


You will be sent a Health Questionnaire prior to your appointment date to complete and return.

This covers relevant aspects of your general health, highlighting any areas which may cause a problem such as allergies.

This questionnaire is crucial in providing us with all the information that enables us to provide you with the right care.

Please remember to return your health questionnaire prior to your clinic appointment.

Pre Admission Assessment


You may be required to attend a clinic before coming in for your treatment, or you may receive a telephone call from a nurse requesting some health details.

These are to ensure that you are well enough for your surgery and to confirm that you understand everything in relation to your hospital stay.

We will also discuss your actual procedure and the best way to aid recovery.

If attending a clinic, we may carry out some routine tests. After the tests you will be able to get on with your normal activities for the rest of the day.

Getting ready for your treatment


There may be certain preparations that you must make before your treatment such as avoiding food or drink for 4-6 hours before a general anaesthetic.

You will receive a letter shortly before you are admitted outlining specific advice on this matter. If by chance you do not receive a letter, please call the hospital you are attending for advice.

If you become ill before you are admitted, please contact us to let us know as it may affect your planned treatment.

What to bring with you


During your stay we will provide you with all the conveniences you would expect, such as fresh linen, hand and bath towels.

We advise you to bring the following items for your stay;
  • Toiletries
  • Hairbrush, comb and shampoo
  • Toothbrush & toothpaste or denture cleaner
  • Shaving equipment
  • Sanitary wear
  • Nightwear and underwear
  • A pair of slippers and a dressing gown
  • Casual, loose clothing to wear during the day
  • Any medication prescription and non-prescription
We advise against bringing valuables, but if you do bring valuable items, they can be deposited with a nurse for safekeeping.
